For this assignment, students will practice documenting counseling sessions with a client. The documentation should demonstrate the ability to appropriately record/document acounseling session of high quality that could be entered into the client’s permanent record.
Students will practice using the Subjective-Observation-Assessment-Plan (SOAP) format for this documentation. Students will watchthree videos online and write a SOAP note documenting each one separately and will submit these to demonstrate their ability to gather information, synthesize it, and write a professional piece of documentation.
S: Subjective: How does the client describe their problem? This is usually a quote or statement from the client describing their subjective description of the problem.
O: Objective: What did you observe about this client? These are written as factual notations.
A: Assessment: What is your impression about/of this client?
P: Plan: What is your plan with this client?
The following is a good example of a SOAP note. Your notes for this assignment should be more detailed that this one, but this shows you how to get started.
